Sue Moreland, MSW, LICSW, is a provider for Lyra Health and Wellspring EAP companies.

Kirkland Psychotherapy PLLC will bill your insurance company for you, in or out of network.

If you have insurance coverage, please contact your insurance carrier to verify your out-of-pocket costs. These can include deductibles, copays and co-insurance. The phone number is usually on the back of your insurance card. Kirkland Psychotherapy PLLC does not do this for you.

Ask the following questions:

• Do I have coverage for outpatient mental health services?
• Do I have coverage for couples or family counseling and under what circumstances?
• What is my copay or co-insurance for mental health services?
• If my therapist is a preferred provider, what are my out-of-pocket costs for an evaluation and for each session?
• If my therapist is not a preferred provider, what are my out-of-pocket costs?

Copayments, co-insurance and deductibles are due at the time of service.

We will work together to create a clear financial agreement as we begin.
